SERVICE CHARGES

  1. Access Line Work

The connecting (splicing) and running of a new number/pair from the customers' pedestal to the customers' protector/NID.

  1. Trip

When a customer trouble report requires a visit to the customer premises by a Cooperative repairman and it is found that the trouble is in the customer provided equipment, a non-recurring Trip Charge will apply whether or not the customer provided equipment or inside wiring is legally connected to the facilities of the Cooperative in accordance with the provisions of Perry-Spencer’s Tariff. The trip charge is also applicable when an employee is dispatched to a designated location to complete a customer service request for the installation, move, or change of service equipment.

  1. Move and Change

For rearrangement of drop wire and /or protector, a Service Order Charge and Trip Charge as specified above shall Apply.

TERMINATION

When a customer cancels an order for service prior to the establishment of service or the expiration of the initial contract period, a Termination Charge may be applicable.

RETURNED CHECK

The Cooperative will assess a charge for each instance where a check is returned or otherwise dishonored by a bank or equivalent business.

Rates and Charges

The Service Charge per occurrence is $15.00 plus any other charges assessed to the Cooperative by the financial institution will be applied to each check returned due to insufficient funds.

RESTORATION OF SERVICE

When service is temporarily suspended for non-payment of charges, the service will be restored upon payment of past-due charges. In addition, a Restoration of Service Charge will be applied.

Rates and Charges

The business and residential rate for restoration of service is $25.00.

LOW-INCOME PROGRAMS

Low-Income Assistance consist of two programs, Lifeline Assistance and Link Up Assistance. The Company participates in both of these assistance programs to increase the availability of telecommunications services to all consumers in its serving areas.

Definition The following terms shall be defined as follows :

Qualifying low-income subscriber – a subscriber who meets the low-income eligibility criteria established by the Indiana Utility Regulatory Commission: Medicaid; food stamps; Supplemental Security Income (SSI); federal public housing assistance or Section 8 (a Federal Housing Assistance Program administered by the Department of Urban Development);  Low-Income Home Energy Assistance Program (LIHEAP); Temporary Assistance for Needy Families (TANF) or National School Lunch free lunch program.

In addition, a consumer may be eligible if his or her household income is at or below 135% of the Federal Poverty Guidelines.

Toll Blocking – a service provided by carriers that lets consumers elect not to allow the completion of outgoing toll calls from their telecommunications channel.

Toll Control – a service provided by carriers that allows consumers to specify a certain amount of toll usage that may be incurred on their telecommunications channel per month or billing cycle.

Toll Limitation – denotes both toll blocking and toll control.

The structure of each program is outlined in the following paragraphs.

 

Lifeline Assistance

General

Lifeline Assistance reduces an eligible customer’s monthly Federal Subscriber Line Charge and rates for local service. An eligible customer receives credit for the Federal Subscriber Line Charge as well as a credit towards the residential local exchange access line rate.

Regulations

  1. Lifeline Assistance is available to all residential customers who meet the following eligibility requirements:

  1. Customers must be participants in one of the following programs: Medicaid;food stamps; Supplementary Security Income (SSI); federal public housing assistance or Section 8 (a Federal Housing Assistance Program administered by the department of Urban Development); Low Income Home Energy Assistance Program (LIHEAP); Temporary Assistance for Needy Families (TANF) or National School Lunch free lunch program.

  2. In addition, a consumer may be eligible if his or her household income is at or below 135% of the Federal Poverty Guidelines.

  1. As a participant in Lifeline Assistance, customers are eligible to receive Toll Blocking Service, as described in their Indiana Serving Tariff, at no charge. These services will only be provided at the customer’s request. The FUSC (Federal Universal Service Charge) will not apply to customers participating in this program.

  2. Local service deposit requirements will be waived for customers who voluntarily receive Toll Blocking Service or Toll Control Service.

  3. Participants in Lifeline Assistance shall not be disconnected from Local Service for non-payment of toll charges. In addition, Perry-Spencer RTC will not deny re-establishment of local service to customers who are eligible for Lifeline Assistance and have previously been disconnected for non-payment of toll charges.

  4. Partial payments that are received from Lifeline customers will first be applied to local service charges and then to any outstanding toll charges.

    Credits

    The following credits will apply for each customer eligible for Lifeline Assistance:

Monthly Credit 1. Federal Subscriber Line Charge Credit $6.50
2. Residential Local Exchange Service Credit $1.75

 

 

Link Up Assistance (Lifeline Connection Assistance)

General

Link Up Assistance is described as an assistance program for qualifying low-income consumers that consists of a reduction in the customary charges for commencing telecommunications service and a deferred schedule for payment of charges assessed for commencing service for which the consumer does not pay interest. All eligible customers receive a credit for half of the service connection charges up to $30.00 and do not pay interest on connection charges of up to $200.00 that are deferred for a period not to exceed one year.

Regulations

  1. The same eligibility requirements as outlined for Lifeline Assistance will apply to Link Up Assistance.

  2. This credit is only available on the installation of a single residential access line at the principal residence of the eligible customer.

  3. This credit will apply for a second or subsequent time only for a principal place of residence with an address different from the residence address at which the Link Up Assistance was provided previously.

  4. An eligible low-income consumer may choose either or both the Service Connection Charges Credit and the Deferred Payment Schedule.

 

Service Connection Charges Credit:

Half of Service Connection Charges or $30.00, whichever is less.

Deferred Payment Schedule

A deferred schedule (not to exceed a period of one year) for payment of the Service Connection Charges up to $200.00, for which the consumer does not pay interest. These Service Connection Charges do not include any permissible security deposit requirements.

PREFERRED INTEREXCHANGE CARRIER (PIC) CHANGE CHARGE

General

A PIC Change Charge will apply whenever a customer requests a change in their preferred interexchange carrier for either interlata or intralata long distance service.

Rates and Charges

  1. A PIC Charge of $5.50 will apply to each customer’s request for a change in their interlata or intralata preferred interexchange carrier.

  2. If the customer requests a change in both their interlata and intralata preferred interexchange carrier on the same order, only one (1) PIC Service Order Charge will apply.

  3. A PIC change charge will apply to each telephone exchange service line or trunk requested by the customer as set forth in the National Exchange Carrier Association’s (NECA) Tariff F C C No. 5, Section 17.4.4

  4. No other service charges will apply in addition to those charges outlined above.

  5. If a preferred interexchange carrier elects to pay those charges for changing a PIC on a customer’s behalf, those charges will be billed directly to the customer by the telephone company. It will be the responsibility of the preferred interexchange carrier (s) and the customer to agree upon how those charges will be paid to the telephone company.

  6. If a customer requests other services as prescribed in the telephone company’s Tariff at the same time as a change in their preferred interexchange carrier, then those service charges as outlined in the Tariff will apply in lieu of the PIC Change Charge. The NECA PIC Change Charge will apply for each telephone exchange service line or trunk having the interlata carrier switched.

LOCAL EXCHANGE RATES

General

Basic Local Exchange Service is provided by means of station, wire, switching and other facilities, plant and equipment to enable the establishment of telephone communications between stations in the same or different exchanges at monthly rates as set forth below. The facilities, plant and equipment used to provide Basic Local Exchange Service are also used in the furnishing of toll telephone services at rates applicable for such services.
